What to bring to your appointment

  • A referral from your GP or Specialist.
  • Your Medicare Card, DVA card and pension card, if appropriate.
  • Private hospital insurance details, if appropriate.
  • Any relevant x-rays, CT scans, MRI scans or ultrasounds.
  • List of your current medications and their dosages.

Patient Registration Form

First time patients will need to fill out a Patient Registration form. This is available below to fill out and bring to your appointment. Alternatively, please arrive at your initial consultation 10 minutes early to fill this out at the practice.

Current Referral Forms

New patients must have a current referral from either their general practitioner or specialist. GP referrals are generally valid for 12 months and specialist referrals are valid for 3 months. A referral is necessary to claim a rebate from Medicare.

Worker’s Compensation Patients

For patients under Worker’s Compensation schemes a referral is necessary. In addition, approval for the consultation will need to be arranged with the insurer prior to the appointment. Please note that in addition to filling out the standard Patient Registration form below, the Workers Compensation Pre-appointment Questionnaire (below) also needs to be filled out prior the consultation.

Uploading your Referral Form

Please provide us with a copy or photograph of your referral form via our ‘Upload Zone’ on this website. You are also able to upload the Workers Compensation Questionnaire and any other documents that Professor Owler may need, prior to your appointment.

Payment methods

We accept payments made with cash, Mastercard, Visa and EFTPOS (No American Express). Please do not hesitate to contact us if you have any enquiries regarding fees and payments.

Cancellation policy

In the event that you need to cancel or change your appointment, please let us know at least 48 hours before. This will enable us to offer your appointment to someone on the waiting list.
